You and/or your legal representative(s) have entered into a cooperative partnership with BridgeStone’s providers and staff. As you interact with each other in this partnership, there are rights that you have and responsibilities that both groups must be aware of and fulfill. Examples of these rights are as follows: Client inclusion in accurate decision-making about your healthcare, client respect, and considerate treatment regarding the privacy of your care and health information being relayed to designated individuals involved in your care.

As a member of our practice, you have the right to
- Have privacy as needed to maintain and respect your dignity, values and beliefs.
- A provider who will not withhold any known or available communicable information about your diagnosis, treatment options, alternatives, risks involved, and possible prognosis to you in a clear and understandable terms.
- Ask your provider about the choices you have and participate in decisions regarding your treatment plan.
- Identify and explain uncomfortable and unpleasant information or clear any doubts about the care we provide in case of any safety concerns as it relates to your mental wellness.
- The right to change your providers if other qualified providers are available for you.
- Know the name and credentials of the professional who renders services to you or with whom you interact.
- Expect effective communication and strict adherence to your confidentiality.
- Withdraw your consent for treatment any time as provided by law.
- Expect confidentiality of your medical record and billing information to the extent provided by law, which is detailed in BridgeStone’s Notice of Privacy Practices
- Request that a copy of your medical record be provided to you or a third party.
- Expect a safe and secure virtual or in-person environment while receiving care that is free from any form of discrimination.
- Request an explanation of all charges, payment policies and billing procedures.
- Expect a timely resolution of your health care and/or billing concerns.
- Receive disclosure if you are referred to entities in which BridgeStone has a financial interest.
- File a complaint, grievances, or concerns or give feedback regarding the care and service received without fear of being discriminated against by communicating your concerns directly with a staff member by sending a message to the Office Manager through your Patient Portal or by emailing us at [email protected].
- If you believe that BridgeStone has violated any of these rights or other civil rights to which you are entitled, you may submit a complaint to the Department of Health & Human Services. Information for submitting such complaints may be found on the Department’s Complaints & Appeals page.
As a member or a member’s legal representative, you have the responsibility to
- Show respect to your provider and other members of BridgeStone MWT’s staff with whom you communicate. Verbal abuse, including profanity and discriminatory or threatening language directed toward any BridgeStone MWT employee will not be tolerated and may result in the termination of our relationship.
- Provide us with a complete and accurate medical history. This history includes all prescribed and over-the-counter medications you are currently taking or have recently taken.
- Tell us about all treatments and interventions you are receiving.
- Follow the suggestions and advice your healthcare providers prescribe in a course of treatment to help your provider provide safe care. In the event that the client’s refusal to declare other treatment prevents us from providing the required ethical standard care, BridgeStone may terminate our working relationship with reasonable notice.
- Ask your provider any questions you may have relating to your treatment.
- Report any health issues, worsening symptoms or medication reactions to your healthcare provider promptly.
- Participate with your provider in deciding on your plan of care, and then follow that plan.
- Use any prescribed medications only for you (the member) exactly as instructed by the provider and contact your provider with any questions about your medication plan.
- Know what your insurance or health plan covers.
- Meet and make any financial obligations agreed to with BridgeStone and/or your insurance company, including co-pays, to avoid service interruption. It is the client’s responsibility to provide BridgesStone MWT with any payment update/correct information about your bill payment, keep your scheduled appointments and, when necessary, follow the cancellation policy to avoid no-show and late cancellation charges.
